Seriously he come in out of shape? He didnt produce until the 4th preseason game last year and sat all season. Clay should easily push him off the roster.

ADD can have a lot to do with it. He may wake up every morning bound and determined to make this the day he begins his training, only to go to bed that night wondering what happened to the day and even more determined to start tomorrow.
That being said, there comes a point where you reach out for help. Ask a team mate or a coach or higher a personal trainer to come and "encourage" you. Stop by the house to pick you up, etc. etc.
